Both bacteriostatic water and sterile water for injection are purified water. But they behave very differently once you open a vial, and that difference is the whole point.
Sterile water
Sterile water for injection is purified water with nothing added. It is designed for single use only.
Once you break the seal, there is nothing inside to stop bacteria from growing. Because of that, any unused water left in the vial must be thrown away right away.

Bacteriostatic water
Bacteriostatic water is sterile water with about 0.9% benzyl alcohol added. Benzyl alcohol is a preservative, meaning a substance that keeps things from spoiling. The word "bacteriostatic" simply means it slows or stops bacteria from growing. Think of the preservative like a security guard posted inside the vial, keeping bacteria from taking over after you open it.
Because of that preservative, a multi-dose vial (a vial you can draw from more than once) can be opened and used again over a period of time, rather than thrown away after a single use.

Why the preservative matters for multi-dose use
The benzyl alcohol is what allows you to go back into the same vial more than once.

What the research community gets wrong about bacteriostatic water vs sterile water
These two diluents look identical in the vial, so it is easy to mix up how they behave at the bench. Here are the points people get wrong most often.
- They are not interchangeable once a vial is open. Both start as purified water, but sterile water for injection has nothing added and is meant for a single use. Bacteriostatic water has benzyl alcohol added, which is what lets you go back into the same vial more than once.
- Bacteriostatic does not mean sterile-forever. Benzyl alcohol slows or stops bacteria from growing. It does not kill everything, and it does not make careless handling safe. Clean technique with the vial and stopper still matters every single time you draw from it.
- The preservative has more than one strength. Labels list either 0.9% (9 mg/mL) or 1.1% (11 mg/mL) benzyl alcohol. Do not assume every vial matches. Read the number on the product you actually have, because benzyl alcohol can interact with some sensitive samples.
- Open vials do not last forever. A common lab practice is to discard a multi-dose vial about 28 days after it is first opened. That is a practice, not a promise printed on every label, so check the label you have.
- Storage rules are not optional. Keep vials at controlled room temperature (68 to 77 degrees F) and do not freeze them. Freezing is a common mistake that people assume is harmless.
